Any diver wishing to log dives for the purposes of APEL, must submit their dive log on these sheets.  Ensure that all columns have been filled and that all gas and working calculations have been completed.  Incomplete log sheets will not be accepted.
You can download and print off log book pages here to complete yourself.  Downloads are free, and once a page is complete, it should either be inserted into your log book record folder or your Training Portfolio.

Diver's Logbook:

The accurate and daily completion of a dive record log will satisfy the requirements of the Diving at Work Regulations 1997 Regulation 12(3)(a) for the diver engaged in a diving project to maintain a daily record of their diving activity. 

Regulation 12(3)(b) requires divers to keep a daily record of their diving in their posession for at least two years after the date of the last entry.

The SDSC has produced a logbook to meet the specific needs of scientific and archaeological divers who are conducting at work diving operations. The books are A5 sized (mm 160w x 215h x 18d) and are hard bound. Inside the front cover there are pages to store personal data as well as guidance on how to comply with maintaining a dive log under the 1997 UK Diving at Work Regulations. Each double page opens to give a blank page for noting scientific or archaeological observations and a page to log the details of upto three different dives. 

In total there are 88 dive log pages so each logbook can record the details of upto 264 dives. The dive log is broken down into sections designed to ensure that all the relevant information required under DWR97 is logged. In addition, at the back of the book there is useful information relating to wind and fog scales and there is also a plastic wallet attached inside the back cover for easy storage of current certificates.
